What Is Liposuction?
Liposuction involves the removal of excess fat from the body. It is a surgical procedure that can be done under local or general anesthesia. Liposuction has evolved greatly over the past 30 years. When it was first developed, it was difficult to achieve good results and bleeding after surgery was often a problem. General anesthesia was required and blood transfusions were often necessary. In 1987, Dr. Jeffrey Klein, a dermatologist, described the tumescent technique for liposuction. This technique revolutionized liposuction by making the procedure easier to perform with few complications. Tumescent anesthesia consists of an infusion of low-concentration anesthetic and epinephrine (a blood vessel constrictor, also known as adrenalin). The fluid plumps up the fat cells and drastically reduces bleeding. Liposuction could now be performed under local anesthesia, safely, and in an outpatient setting.
TUMESCENT ANESTHETIC INFUSION
In all forms of liposuction, fat is removed with the aid of a cannula and suction. After the tumescent anesthetic is infused through tiny skin incisions, a suction cannula is placed under the skin. The cannula has holes on it through which fat enters and is sucked away through tubing to a canister. If fat is being saved for fat transfer, then a specialized container is used. Fat is gradually removed with smooth back and forth motions of the cannula. The surgeon pays attention to even removal throughout the area treated.
An ideal candidate for liposuction is not obese but, rather, has specific areas of increased adiposity that have been resistant to diet and exercise. Liposuction is really body contouring, bringing those problem areas into line with the contour of the rest of the body.

New Technology in liposuction
At Celebrity Laser Spa in Los Angeles, the form of liposuction offered is called Tickle Lipo. Tickle Lipo is a kind of power-assisted liposuction. That means that the cannula is powered to vibrate, thus removing fat more effectively and safely. Rather than relying on the surgeon's brawn to push the blunt cannula through the fat, the vibration of the cannula let's it "swim" through the tissue and gently remove fat with less trauma to the skin. Thus, less bruising and post-operative pain is typical and patients have a smoother, more even result.
Other recent liposuction technologies include ultrasound and laser. Ultrasound cannulas are solid cannulas that emit sound waves powerful enough to melt fat. After melting the fat, a suction cannula is used to remove it. Vaser is a type of ultrasonic liposuction technology. Laser lipo substitutes a laser for the same process. First, fat is melted, then it is removed with a suction cannula. In Dr. Kaplan's opinion, both of these techniques have certain disadvantages compared to Tickle Lipo. They both entail the use of energy (light or ultrasound) to melt fat. This creates an additional variable for the surgeon to worry about when trying to achieve a smooth and even result. The use of heat creates the risk of burns to the skin and deeper tissues. Also, since liposuction is really "liposculpture", it is very difficult to know exactly how much fat to melt before it is time to remove it. With Tickle Lipo, fat is removed "on the go" rather than at the end of the procedure. Therefore, a smooth and even result gradually takes shape during the procedure. Also, fat that has been heated or destroyed by laser or ultrasound cannot be used for fat transfer to other areas.
